Data audit report question

Hi,

I have a data audit report:

If I read this correctly it says that the value highlighted in yellow was entered into the system using a specific data entry form. Is that interpretation correct?

Because if it is, it makes no sense. The form mentioned doesn't allow input into the parameters that the highlighted row uses. We have specifically tested this in that myself (different user) and the original user opened the form and tried to change the value. We both got a message saying that no value was changed (because the cell is locked).

So, if the audit report is correct but the cell on the form is locked, how else could the data have gotten into the database? Or is the audit report incorrect and shows the wrong source?
